Liability Waiver & Medical Release
Please read carefully before completing your registration. By submitting payment you confirm that you have read, understood, and agree to all terms below. This agreement is entered into voluntarily and is legally binding.
For the purposes of this agreement, “Released Parties” means Life Skills Camp, its founder, organizers, volunteer instructors, staff, agents, the owners and occupants of the session venue, all members of the hosting household, and each of their respective heirs, successors, and assigns. “Parent” means the parent or legal guardian completing payment. “Participant” means the registered minor. By completing payment, Parent agrees to all of the following on behalf of themselves and the Participant:
Acknowledgment of Activities
Parent understands that Life Skills Camp sessions involve hands-on household activities including, but not limited to, operation of washing machines, dryers, dishwashers, irons, vacuum cleaners, and the use of cleaning products and chemicals. Parent acknowledges that these activities carry inherent risks of injury.
Assumption of Risk
Parent knowingly and voluntarily assumes all risks — known and unknown — associated with the Participant’s attendance at and participation in Life Skills Camp sessions, including but not limited to physical injury, illness, property damage, or any other harm arising directly or indirectly from participation in program activities, travel to or from the session venue, or presence on the session premises.
Release of All Claims
In consideration of the Participant’s admission to and participation in Life Skills Camp, Parent, on behalf of themselves, the Participant, and their respective heirs and legal representatives, hereby fully and irrevocably releases, waives, and discharges the Released Parties from any and all claims, demands, damages, actions, causes of action, or suits of any kind — whether known or unknown, anticipated or unanticipated — arising from or related to the Participant’s participation in Life Skills Camp, including any claims arising from the negligence of the Released Parties.
Indemnification
Parent agrees to indemnify, defend, and hold harmless the Released Parties from and against any and all claims, liabilities, damages, losses, costs, and expenses (including reasonable attorneys’ fees) brought by or on behalf of the Participant or any third party arising from the Participant’s participation in Life Skills Camp.
Supervision Acknowledgment
Parent acknowledges that a responsible adult will be present at all sessions. Parent further understands that the program takes place at a private residential property in Alexandria, Virginia, and that the venue is a private home — not a licensed childcare or recreational facility.
Voluntary Participation
Parent confirms that participation is entirely voluntary and that neither Parent nor the Participant has been coerced or pressured into registering for or attending Life Skills Camp.
Authority to Execute
Parent represents and warrants that they are the legal parent or guardian of the Participant, that they have full legal authority to execute this agreement on behalf of the Participant, and that the Participant is a minor under the age of 18.
Binding Effect
This agreement is binding upon Parent, the Participant, and their respective heirs and legal representatives. It shall remain in full force and effect for all Life Skills Camp sessions attended by the Participant.
Severability
If any provision of this agreement is found to be invalid or unenforceable by a court of competent jurisdiction, the remaining provisions shall remain in full force and effect.
Governing Law
This agreement shall be governed by and construed in accordance with the laws of the Commonwealth of Virginia. Any disputes arising from this agreement shall be subject to the exclusive jurisdiction of the courts of the Commonwealth of Virginia.
Accuracy of Medical Information
Parent represents and warrants that all medical information provided during registration — including allergies, medical conditions, medications, and physical limitations — is accurate and complete to the best of their knowledge. Parent agrees to notify Life Skills Camp of any changes to the Participant’s health status prior to the session date by emailing lifeskillscamp26@gmail.com.
First Aid Authorization
Parent authorizes any staff member, instructor, or supervising adult associated with Life Skills Camp to administer or arrange for the administration of basic first aid to the Participant in the event of a minor injury or illness during the session.
Emergency Medical Authorization
In the event of a medical emergency in which Parent or the designated emergency contact cannot be reached in a timely manner, Parent hereby authorizes Life Skills Camp staff and/or the supervising adult to: (a) contact emergency medical services (911); (b) consent to emergency medical examination, treatment, and procedures on behalf of the Participant; and (c) authorize transportation of the Participant to the nearest appropriate medical facility. Parent agrees to bear all costs associated with such emergency treatment and transportation.
Emergency Contact
Parent has provided a valid secondary emergency contact during registration. Parent confirms this person is aware of their designation, is authorized to make decisions on behalf of the Participant if Parent cannot be reached, and will be reachable on the day of the session.
Good-Faith Medical Response
Parent agrees that the Released Parties shall not be liable for any actions taken in good faith in response to a medical emergency involving the Participant, including any decisions made while unable to reach Parent or the emergency contact.
Parent grants Life Skills Camp and its representatives an irrevocable, royalty-free license to photograph, film, and record the Participant during sessions and to use such images and recordings for any promotional, educational, or marketing purpose, including but not limited to the Life Skills Camp website, social media accounts, and print materials. If Parent does not consent to this release, they must notify Life Skills Camp in writing at lifeskillscamp26@gmail.com prior to the session date. Opting out will not affect the Participant’s registration or participation in any way.
Note: This waiver is entered into by completing payment on the registration page and is legally binding upon submission. These terms apply to all Life Skills Camp sessions attended by the registered Participant.
